These are traditional and multifunctional tunneling knives.

They were designed to perform full-thickness flap elevation in different areas of the periodontal and peri-implant structure. One of the active tips is straight, working in the anterior area of the maxilla and mandible. The opposite tip is angled, allowing working with a lateral approach of the hand. The size variation between model n.1 and model n.2 allows the natural adaptation to access different phenotypes and teeth sizes. With a highly delicate tip, the smaller instrument can access even the most delicate lower incisors.

This tunneling instrument accumulates two different functions:

First is to act as a “syndesmotome,” performing the rupture of the fibers of the gingival sulcus in a highly delicate way, including the interproximal area, which helps in the papillae structure preservation when doing interproximal tunneling or in atraumatic extractions.

 

The second function of the Tunneling Spatula is to help prepare the flaps for coronary advancement, separating the mucosal tissue layer from the muscle bundles in a spotless manner. This action improves flap releasing, increases passivity, and produces a surgical recipient bed without the presence of interposed muscle groups.

This is a fantastic and unique instrument with no equals anywhere in the world! It makes the detachment of the papillary base of the flap much safer, faster, and more efficient, reducing the rate of papilla ruptures at the time of elevation.

Its unique shape and curvature allow the penetration through the sulcus without stretching the margins, while the active part can go under the buccal base of the papilla, promoting slight leveraging movement and detaching the tissue from the ground until the opposite sulcus.

An instrument that combines dual function with different function tips.

 

One tip features a design for lingual tunneling  (although not very common in our daily office routine, lacks instruments with the design for proper execution).

The rounded delicate active tip on top of an extended straight shank, with the bending part closer to the base, allows greater accessibility and safety when lifting the thin lingual tissue.

 

On the tip of the opposite side, we designed a tunneling instrument to perform the last step in tissue preparation: the releasing of the papillae in the crest top area. 

It is necessary to remind that this procedure should be limited to the assessment of the papillary width; extremely narrow papillae should not receive total detachment of their base from the bone peak, under the risk of increasing cases of papillary necrosis. On the other hand, when dealing with patients with wider papillae, we can release the tissue over the bone peak with reduced rates of surgical necrosis during healing; however, delicate surgical manipulation is essential. This tunneling instrument has the ideal design to promote papilla detachment with minimal tissue trauma.
